Does anyone have a sample contact sheet they use for their unit owners to complete? And do you have a separate one for renters? We are a 33 unit building and people don't let the management company know they are renting and in some cases then we don't know who is in the unit. I was thinking of sending everyone a contact sheet to complete with a questionaire.

Any samples or suggestions?

I take it you are a Condo, is this correct? How old is your complex. are your units prorated for assessments? What have you done in the past? I noticed you said something about a management company. Do you rely on a M/C for a complex of 33 units? Where is your board, they hired the M/C to manage I suppose. Are there any provisions for rental property in your documents? Does the Board have a rental policy? Does the association have right of first refusal when propertyy is sold? It would do you no good to send out contact sheet requesting information if you did not have policy and authority in your documents. I know it is common for the Board NOT to know who is living in the units, however, many associations have covenants that make it a requirement that the owners must copy the Board on all rental agreements, must provide a list of all occupants, just like a motel or hotel must register quests. Why all the interest in this practice now? What else is going on? How many of the 33 owners rent? Have you had any Mortgage Companies refuse to provide a mortgage because of the number of rental units.

We use the same form for both owners and renters. Ohio condo law requires they provide the info within 30 days of move in although there are no penalties if they don’t comply and it’s up to the Association to see that it gets filled out.

I don't actually have a sample to show you but we just have an emergency contact form for all homeowners asking name unit# and 2 emergency contact phone numbers Then below its asks do you have a tenant and if yes we ask for tenants name and 2 emergency phone numbers We have problems getting some of the homeowners who live here to fill them out but the ones who have tenants have sent in the info since they are not here to watch their property and want someone contacted in case of emergency

My assn has a rental agreement that the property owner must sign and abide by. I know many assns go this route. If the property owner fails to sign the rental agreement, fines are imposed just the same as they are imposed for any other CCR violation. There are 1,702 members in my assn and we do not maintain an emergency contact list. Many small assn's may go this route; but it would be quite cumbersome for a large assn to maintain.
