We are an association management company and would like to know if anyone has an evaluation we can distribute to board members and owners of the association to assist us with our goals and overall management performance.

Does anyone have a template or know where I can locate one?

I don't have a template, but you could start with considering what your management company is supposed to do and then ask homeowners things like:

Did you contact the property mananger at any time during this year?
If so, what was the request?
How long did you wait before your request was granted?
If your request wasn't granted, why not?

This is off the top of my head, but you can tailor this to see how quickly the company responded to maintenance requests, whether workers did the work to their satisfaction, damaged any personal property, etc.
