check with your local register of deeds or where ever liens are filed to see what they accept or need. I was given examples of what other people did in my location and we created our own form.

Hi Les.....
Where I live in Florida the court will only accept a lien form from an attorney...save yourself some grief and call the clerk of the court.

Not true. A person can go to small claims court without an attorney and can certainly file a lien without an attorney. Ask a lot of subcontractors who file their own liens without the benefit of an attorney. Where do you live in Florida and who gave you this advice?
