We have a beach and boat well for use by Members of our HOA. We do not have any kind of hold harmless form for members to sign and have been advised by our insurance company to have one on our membership application.

Any of you wonderful people have a sample hold harmless signature requirement?

If your insurance company suggested this, why didn't you just ask THEM for a sample and then run it past your association attorney to ensure it'll address whatever's in your state law and master insurance policy?

You might also try Google - here's a site with some samples: http://www.moperm.com/Resources/FORMS/FORMS02.pdf

These probably don't fit exactly what you may need, but the idea is to get some insight as to how they're crafted so you can have a productive discussion with your association attorney and insurance company. It's never a good idea to cut and paste stuff like this.

Here's the one we use for our community boat slips.
