Does anyone have a sample of the election forms and proxy forms for HOA elections, or can give me an idea of where to fine a sample. We are getting ready for our first elections and have nothing to follow.

I was unable to attach a copy of the ballot to this form, or to copy and print it here.
Send me your E-mail address and I can attach it. Our ballot is used for in meeting voting
and mail in balloting.

Check your state laws regarding proxies (usually found in your state laws regarding corporations). Although you won't likely find sample forms, you may find what is required to be included in the proxy appointment and that would govern what needs to go on your form. Some states limit the duration of proxy appointments, other states may allow you to establish the duration in your bylaws or on the appointment form.

An example of a candidate form.I suggest you allow absentee voting and not allow proxies.
