Looking for a simple system where homeowners can fill in an Architectural Change Request online.
What we have now requires downloading, filling out, scanning, and uploading.
Any suggestions on what we should use or look for?

People have put together online forms with Word or Access - if you know someone who's savvy with those programs, this could be a quick and relatively inexpensive way to get started. He or she may also be able to figure out how to set it up so people can upload supporting documents with their requests. I'm no expert, but I would think Access might be good for this, as you can use it to store data for later use in reports.

I use a program called PowerPDF 2.0 that creates a fillable form and I add a submit button which will automatically send to an email of our choosing.

If they have plans that need submitting , they would still need to be uploaded. All my forms can be completed online without having to download.

Adobe Acrobat (Pro) will also allow you to make fillable forms. Personally I would not build emailing into the form, simpler just to email the form when done.

PowerPDF - $129 - Requires technical knowledge, can be complicated.
Acrobat Pro - $450 - - Requires technical knowledge, can be complicated.
(Fillable PDF's may not work on phones without installing extra software)

Google Forms - Free - No software needed. Simple. Can attach photos or files. Works on iphone, android, pc, etc.
(Does not need extra software for homeowners to fill out. Works on any phone, computer, etc with a web browser.)

Changing directions. Now looking for new website application.

In addition to online ACR entry, we want to be able to track certain information by household.

For example, we have do not prune, do not mulch, and do not use pesticides lists. Pesticides is just a check the box item. But for pruning, homeowners can select what doesn't get pruned by specific location.

Most website options we've looked track things like vehicles, residents, and pets, but don't allow us to create our own categories.

Any suggestions or recommendations.

Google Sheets - FREE
Its simply just like Microsoft Excel, but online and publishable. Make as many categories or locations as you want.
