Transfer fee invoice template?

I just took over as treasurer of our small HOA. It appears at the last board meeting they approved a new transfer fee (just $35) for when a home is sold. That said, is there a specific format or template that people use for this? Since this is the first one since the fee was instituted, I have nothing to look back on and am unsure of what all it needs to say or include. Any guidance or an example would be appreciated. Thanks!
