attached are two very basic proxies.
One is a general proxy - Proxy is assigned and allowed to vote as they think is best
One is a directed proxy - Proxy is assigned and instructed to vote a certain way

You should run any form past your Association attorney to ensure that it complies with your state's laws, of course.
I also include in my proxy forms an area where the homeowner granting the proxy is to provide his/her telephone number and email address. That tends to curtail the shenanigans that sometimes occur with the use of proxies; it makes it difficult for someone to fabricate a signature of the proxy with such corroborating information, and also tends to check the actions of some power-hungry Boards in disqualifying proxies for questionable reasons.
