Does anyone have an example of a format of a blank special meeting form? Reading back on other postings, looks like it's just a simple form needed for name and address with signatures of homeowners.

Tara, are you asking how to create a petition for the members to use to call a special members meeting? Just state we the undersigned members of ______ HOA do hereby call (or request the President or Board members to call) a special meeting of the member for the purpose(s) of _______ on (date) at (time) at (location).
Leave several blank lines for signature, date, and address. Several sheets of paper can be used to provide to those members who will be going door to door for signatures. After getting more than the minimum required by the By-laws (such as 20%) hand carry the originals to the President or to a Board meeting and keep photostatic copies.
