My neighbor and friend works offshore and would like to give me permission to vote on his behalf at an upcoming special meeting (Louisiana HOA). Our bylaws state:

Each voting member shall be entitled to cast one vote. A voting member may vote either in person or by proxy executed in writing by the member or by his or her duly authorized Attorney in Fact.

How can we go about doing this? How should the form read if he wants to give me his full voting rights at the next meeting?

The HOA should provide you with a proxy form in advance.

All he needs to do is fill it out and sign it saying that you are his proxy and can vote on his behalf.

There is not a standard form.

Essentially a form would read something along the lines of "I __jack johnson____ hereby assign ___john doe___ of 123 main st, internetville, US 12394 as my proxy to vote on my behalf at the 2013 annual meeting of Atlantic Ocean HOA."

However, more concerning, is the fact that a proxy form is not being provided. There are annual meeting notice requirements, and most of them require something along the lines of a proxy to be sent to everyone. Your bylaws should tell you what is required under "annual meeting."
