PUD questionnaires, who charges and how much?

Our Association is getting annoyed with having to fill out various PUD questionnaires. I believe that this is our 5th one in the past 3 months. Some from selling and some for refinancing.

It takes at least 30 min to complete one. How many associations charge a fee to complete and to whom (owner/buyer)?

Has someone come up with a standardized form they are willing to share?

PUD=planned unit development

The questionnaire comes from banks, etc. for the new FHA guidelines. Some of the questions are: # of units, how many rentals, how many owners own more than one unit, is there a master association, etc.

Since I posted this, I've done searches on the web and have decided to adopt what other Associations have done: Make a simple paper with the common questions answered and provide it free of charge and if more info is needed charge a $35 fee to fill out their form.
